FOLLOW THESE SIMPLE INSTRUCTIONS
1.) Select the Co-Ops below you would like to join.
Co-Ops range from as little as $5 up to $2000.
We're serious about helping our members
not only survive, but thrive - together!
2.) Purchase four money orders or
certified checks, for the Co-Op or Co-Ops selected, making one payable to each member listed on the current primary benefit flyer below.
3.) Purchase the corresponding postage stamps for each of the members listed for each of the
Co-Ops selected.
4.) Send ALL funds AND postage stamps to the MONITOR at the following address for distribution.

Upon receipt of all the funds & stamps, the Monitor immediately distributes the applicable funds and stamps due to each qualifying member for each
Co-Op selected by the giving member.
